1. Create a patient type document template. Go to the Menu > Settings > Practice Setup > Document Templates. Click Add New Document Template. Name it and leave the "Template is used for?*" as "-- Not Specified --"
2. Select the settings to Allow edit prior to print/email?* and Store generated document into the patient's medical record? *
3. Copy this source code below, and then access Tools > Source Code > and paste into the blank field. Then click "Ok".

Note: You may also set restrictions on which employee is allowed to generate this patient document, based on role or specific account. Enable "Restrict employees?" > Add employees that will be allowed to generate/edit this document.
4. Scroll to the bottom of the page and click Save.
